Proof of residency (lease agreement or recorded deed)
Current utility bill (water, power, internet, or gas)
Birth certificate
Parent/Guardian Photo ID
State of Alabama Immunization Record
Withdrawal slip from the previous school (grades 9–12)
Custody documents (if applicable)
