• Welcome to South Shades Crest Elementary!

    We are thrilled that your child will be attending South Shades Crest for the 2022-2023 school year! It is going to be an amazing year! This is a lot of information.  Please complete this form so that we have your contact information.  You will need to complete additional information below.

    Registration Information: 

    To get your child entered into our official database and placed in a class we will need to collect the following information from you. (If your child is already a Hoover City School Student, First Class PreK, or current SSC Student, you do not need to complete step 1, but are welcome to come for the tour if you would like)

    Step 1: Complete Enrollment Paper Work (for the school cumulative record file)

    • Attend the Onsite Enrollment Event on April 26, 2022 from 3:30-5:30 at South Shades Crest Elementary School

      • At this event, you will complete your enrollment forms. You may print the forms and bring them already filled out if you would like. Enrollment Forms Attached

      •  You will need to bring a copy of the following:

        • Current Lease Agreement or Recorded Deed

        • Current Utility Bill (water, power, or gas)

        • Birth Certificate

        • Social Security Card (voluntary)

        • Immunization Form (Alabama IMMPRINT Certificate)- we will need this prior to school opening, but understand some of you may not be able to get this from your child’s doctor at this time. 

        • Government Issued Photo Identification (for parent/guardian)

        • Custody Papers/Marriage Certificate (if applicable)

      • You will also have the opportunity to have a student-led school tour. Tours will end at 5 pm.

    Step 2: Complete Online Enrollment (to get you entered into the official Database)

    • Mid May the online registration link will go live. I will send you an email letting you know when this link is available for the 2022-2023 school year. Much of the information will be the same as what you turn in to the school.  

    After you have completed Step 1 AND Step 2 your child will be officially registered for school. Congratulations! 

    Class Placement Information:

    I would like to offer parents the opportunity to let us know what environment you feel is most appropriate for their child. While it is not possible for you to choose your child’s teacher, we do want to make sure all of your concerns are addressed when making next year’s placement. If you should have special concerns you would like us to consider,  please note them in the appropriate section provided on this form. 

    You might include a description of the amount of structure/flexibility needed for the learning environment in which you feel your child works best, or if you have had a past experience with a teacher or another student that was either positive or negative. We work very hard to make the best possible class lists and consider all of the given information. 

    Once we begin making class lists, it is difficult to make changes. For this reason, this form must be completed NO LATER THAN May 1, 2022.
