Students requiring special dietary needs will need medical documentation from the attending physician.
It must be provided to the school nurse and should state at a minimum:
- the disability or medical condition that requires students to have a special diet
- the diet prescription
- any foods omitted and substitutions required
A meeting should be scheduled with the parent, school principal, school nurse, a district-level Child Nutrition employee and the student's teacher(s).
Special information regarding milk substitution requests: Per the "USDA Rule on Fluid Milk Substitutes for School Nutrition Programs," our CNP Program does not offer a milk substitution for students with medical or special dietary needs at this time.
Complete and return a Dietary Restrictions Form. Please contact our Child Nutrition Program Department at 205-439-1000 with any questions.
